Has he had all appropriate scans, xrays etc.
If not do this.
My scenario, 59 yrs, never had pain in knees ever, despite skiing for work for 15 yrs.
All of a sudden start feeling pain in r knee, everyone, including doctor, oh you're 59, worn out.
Anyway I go for a few tests, knee looks great ?
Doc says try this, physio, turns out misaligned patella due to muscular strength imbalance, remarkable turnaround with strengthening and stretching.
Stoked.
Anyway just an idea, hope he sorts it.
